Oh, that's easy, it doesn't. You'll have to make Windows dial out to
your ISP before you run PuTTY. PuTTY doesn't use a modem - it's a
network app, so you need a TCP/IP connection up and working before it'll
work. After that, it works just like it does at your work - you just
put the server name in and it connects. If it can - the assumption is
that your work machines will be visible to the internet. If they're
behind a firewall, or not connected to the Internet, then you won't be
able to PuTTY into them anyway.

If you have a dialup system in place at your work, you'll need to set up
a new connection in Windows to dial into that, then make Windows dial
that instead of your normal ISP, then use PuTTY.
